What is Transaxillary Breast Augmentation

Breast augmentation surgery is an excellent option to enhance the shape and size of the breasts. However, many women are understandably concerned about maintaining a natural breast appearance, primarily apprehensive about scarring. With the periareolar and inframammary approaches to breast augmentation surgery, women are left with a scar around the nipple or in the breast fold. Even if these scars are small and well healed, it is an obvious sign of a breast augmentation surgery.

Fortunately, at Ennis Plastic Surgery, we are pleased to offer another an endoscopic approach to breast augmentation surgery: the transaxillary breast augmentation. The transaxillary breast augmentation is performed in an endoscopic manner in which there is no incision (thus no scar) over any region of the breast tissue. The technique is performed through a small incision in each underarm, leaving only a small scar that is hard to detect.

Like other type of breast augmentation surgery, breast augmentation with the transaxillary approach, improves the breast shape by increasing breast volume with implants. This procedure works to enhance a woman’s figure. In addition, the implants work to improve the breast appearance after pregnancy and weight loss. The difference with a transaxillary underarm breast augmentation vs a traditional breast augmentation is in the location of the incision and route in which the implants are inserted.

Is the Transaxillary / Underam Incision Right for You?

During the consultation, you and the surgeon will determine which breast augmentation approach best suits your needs. If the transaxillary approach is most appropriate, the surgeon will review the procedure and expected recovery period. For women requiring a breast lift in addition to a breast augmentation, the transaxillary approach will not be best suited. This is because the breast lift requires an incision on the breast tissue, so the implant can be inserted in an existing incision created for the breast lift.

About the Transaxillary Surgical procedure

The surgeon begins by creating an incision in the armpit, along the area of a natural skin fold. This way, the scar can be well hidden after healing. The surgeon inserts an endoscope, which is a narrow fiber-optic camera through the incision site. This endoscope allows for viewing of the internal anatomical structures and guides the surgeon during the surgical procedure. The endoscope allows for viewing and monitoring, while minimizing bleeding and scarring.

Next, the plastic surgeon creates an implant pocket, typically behind the muscle of the chest wall. The implant pocket is the space in which the implant is positioned. The breast tissue and skin are stretched in order to create this pocket and accommodate the implant.

Then, the implant is inserted through the underarm incision and into the created implant pocket. The surgeon evaluates the size, symmetry and shape of the breasts and ensures that the implant is positioned properly. Once the surgeon is satisfied with the breast implant location and breast appearance, the incisions are closed with sutures, taking great care to create sutures that result in minimal scarring.

No scar on the breast with Transaxillary Augmentation

Endoscopic Transaxillary Breast Augmentation Recovery

Breast augmentation surgery takes approximately one to two hours to complete. The transaxillary breast augmentation is performed under general anesthesia in order to ensure patient safety and patient comfort. Once the surgical procedure is completed and the surgeon is pleased with the breast appearance, the incisions are closed with sutures and the breasts are wrapped in a gauze dressing to protect the healing tissues. Many women are also dressed with a surgical bra for support.

Swelling should be expected, but will rapidly decrease over the first several weeks after surgery. There will be some surgical pain, as the skin over the breasts and the breast tissues have been stretched. However, this discomfort will rapidly subside and can be controlled with pain medications.

Most women can expect to return home after several hours in recovery. Patients are educated in proper care of the breasts and follow up instructions. It is important that patients rest for a few days to allow for full and proper healing of the tissues involved. Exercise and heavy lifting should be avoided for four to six weeks, but patients may return to work and household tasks after one week.
