Facial changes that occur with aging:

There are many signs of aging, but one of the earliest and most visible is the loss of facial fat and collagen. Replacement of volume can take years away from your appearance and can be combined with other facial rejuvenation techniques, like use of neuromodulators and laser or radiofrequency skin tightening. These procedures are much less invasive that the traditional approach with skin tightening by surgical facelift. There are a variety of fillers on the market, and each has been designed with unique properties. Dr. Ennis will select fillers based on the patient’s individual needs and goals.

Hyaluronic Acid Fillers

Hyaluronic acid is a substance found naturally in the soft tissues of the body. In the face, hyaluronic acid is present in tissues and in the periorbital fluid. Hyaluronic acid is part of the dermal matrix, which supports the dermis, a deep layer of skin.

The dermis contains connective tissue, hair follicles, and sweat glands. Hyaluronic acid in the dermis attracts water molecules, increasing the moisture content of your skin. When it is injected into the dermis, it is rapidly cross-linked and incorporated into the skin structure. Unlike naturally occurring hyaluronic acids in the skin, chemically constructed products are longer lasting, although they each have different consistency and duration of effect.

When choosing a hyaluronic acid filler, the skin care experts at Ennis Plastic Surgery consider the particle size, hardness of the gel, and the concentration of hyaluronic acid in the gel. Some commonly used hyaluronic acid products are Juvederm & Perlane. These fillers are temporary, but the effects can last a year or longer, depending upon the product used.

Calcium Hydroxyapatite

Calcium hydroxylapatite fillers include Radiesse. These injectable medications contain small “microsphere” particles of calcium hydroxylapatite suspended in a gel matrix. The FDA has approved Radiesse for restoration of volume in patients with loss of facial fat tissues resulting from HIV infection. Radiesse is also FDA approved for correction of moderate to severe lines and wrinkles, including the nasolabial folds, The duration of these fillers are typically longer-lasting than hyaluronic acid products.

They work by stimulation of your body’s natural collagen production. Microspheres eventually break down into calcium and phosphate ions that are excreted naturally. Studies have demonstrated a duration of effect of up to 18 months after an injection of Radiesse. More than 80% of patients maintained the benefit for longer than 12 months.

Radiesse is a good choice for replacement of volume in large areas like the cheeks. It’s important to choose an experienced medical professional for injection of Radiesse, because it cannot be dissolved if results are unsatisfactory, unlike hyaluronic acid fillers that can be reversed by injection of the enzyme hyaluronidase. Dr. Ennis, medical director at Ennis Plastic Surgery is an expert injector.

Although some patients notice small nodules beneath the skin, this occurs primarily when Radiesse is used for lip augmentation. We do not recommend lip augmentation with Radiesse.

Autologous fat (fat transfer)

Temporary fillers include fat that can be harvested by liposuction from other areas of your body. This eliminates* the risk of an allergic reaction or rejection by your body, but all of the transplanted fat cells don’t survive transplantation so the area is generally overfilled initially.

Rapid resorption will restore in the look you want within a few weeks. Fat injection can result in a very natural appearance and can be used in other parts of your body for volume restoration.

Dermal filler is then injected into areas that have been predetermined before the procedure.

Each injection takes only seconds and the entire procedure will be over in minutes, leaving your with an immediate yet natural improvement in your facial lines and creases. Depending upon the filler used, results normally continue to improve over several weeks.
