Changes in skin elasticity, fat distribution and the tissues beneath the skin can contribute to a sagging neck. Genetics, sun exposure and weight changes also influence what you notice. The concern may be vertical neck bands, fullness beneath the chin, loose skin or a jawline that has become less distinct.

The SMAS is a supporting tissue layer beneath the facial skin; the platysma is the thin muscle beneath the neck skin. These related structures help explain why lower-face and neck changes are assessed together. Treating only the surface may leave an underlying concern unaddressed.

At Ennis Plastic Surgery, the range of approaches includes the Mini-Mini facelift, Mini Deep Plane Facelift, traditional open neck lift and deep plane facelift with neck rejuvenation. The word “mini” alone does not tell you which tissues will be treated or how much change to expect.

Ask Dr. Ennis to explain the proposed incisions, the areas being lifted and whether excess skin will be removed. A limited approach may suit a focused concern, while more extensive jowling or skin laxity may require a broader plan. Suture suspension and deep plane facelift surgery have different scopes and should not be expected to produce the same result.

Developed by Dr. Gregory Mueller, MyEllevate uses the ICLED® light-guided surgical suture system. Illumination helps guide the placement of a continuous supporting suture beneath the skin through small entry points. The suture supports underlying neck and jawline structures; the technique itself does not remove excess skin.

It may be considered for selected patients with early neck changes and limited skin laxity. A preference for a smaller procedure still needs to be weighed against the amount of loose skin, fullness and lower-face descent present. Discuss what suspension can change and what would remain untreated.

The plan begins with an assessment and markings tailored to your anatomy. Local anesthesia, with sedation when appropriate, is discussed beforehand. If submental liposuction is included, fat beneath the chin is removed through a small access incision using a cannula. Light guidance then assists placement of the supporting suture.

The manufacturer describes the suspension procedure as taking approximately an hour, but additional treatments and individual anatomy can change the time needed. Confirm the full operative plan, anesthesia arrangements, surgical setting and aftercare with Dr. Ennis. Small entry points still require healing and carry procedural risks.

Suture suspension may be discussed alongside submental liposuction, eyelid surgery or a treatment for skin quality. Chemical lipolysis, injectable treatments and energy-based tightening address different concerns and have their own suitability requirements and risks. They are not interchangeable with lifting deeper tissues or removing loose skin.

A combined plan should explain the purpose of each treatment, whether it will take place at the same appointment and how it affects recovery. Ask whether staging procedures would be more appropriate. The objective is a coherent plan for your concerns, rather than adding treatments simply because they can be combined.

In the first days, allow for swelling, bruising and follow-up care. A chin strap may be prescribed for part of your recovery; its fit and wearing schedule should come from your surgical team. Return to desk work, physical work, housework and heavy lifting are separate decisions. MyEllevate patient materials describe a return to many usual activities around a week, excluding heavy lifting, but this is not clearance for your individual plan.

Over the following weeks and months, swelling continues to settle and scars mature. Early improvement does not mean healing is complete. If the contour remains a concern, discuss it at follow-up rather than assuming the suture can be adjusted easily or that another procedure will be necessary.

Long-term care includes sun protection, a stable weight and an appropriate skincare routine. Skin treatments such as RF microneedling or other tightening technologies may be discussed individually, with attention to their risks and timing after surgery. No maintenance plan stops aging or guarantees a particular duration of results.
